Depends on the identity.

If the identity were, say, actually tied 1:1 to the individual person sending
the mail then there are some pretty clear routes to severely reducing the
spam issue.

If independent identities can be created in arbitrary amounts at trivial
cost - and that's what most realistic email identity systems are - it's
a bit different.

How the identity is generated seems more important than
the details of how it's communicated to the recipient when it
comes to "how does can it affect spam".
