Just check out the MIB review guidelines:

	draft-ietf-ops-mib-review-guidelines-03.txt

There are pointers to the proper boilerplates. For the editorial stuff,
it should be sufficient to go through the MIB Review Checklist in
appendix A.

Sure, but for the Draft -> Standard transition, we need deployment 
reports not implementation reports. And it should be possible to 
find people on the net who use the BRIDGE-MIB for doing something 
useful with it. A simple post on comp.protocols.snmp plus a few
emails to some mgmt application vendors should get us some input.
