This is more of a T10 question but my take is this: The ASSOCIATION field is 
simply used to show which entity the DESIGNATOR field is associated with. It 
has nothing to do with the PROTOCOL IDENTIFIER. You may get several 
Designators, one for each type. For iSCSI the PROTOCOL IDENTIFIER would 5 
for ASSOCIATION values of 01b or 10b when the PIV bit is set.
