Intermediate snapshot of protocol document

Hi everyone,

I've created an intermediate snapshot "06.a" of the protocol document.
This snapshot is totally unofficial, but I hope it helps in keeping
track of which issues we still need to discuss.

This contains new text about the following issues (in some cases, 
we perhaps haven't fully agreed on the text yet but at least 
there's a proposal):
