In preparation for IETF Last Call, this revision captures the changes
made as a result of Expert Review. I reconfirmed via the list archive
that Dave and Tom are both OK with the new text.

As an additional clean-up, I've re-ordered the IANA and Security
Considerations sections to the usual sequence of Security first, then
IANA.
