bit locations not specified for HdrCt

The bit locations for the M, D, and R bits of the HdrCt field are not
specified in either figure 7 or 8. Based on the order in which they are
listed on page 39 under HdrCt, it is clear that a "normal" engineer
would assume that the M bit goes into the MSB. However, there may be
"MSB challenged" engineers who believe that the M bit belongs in the LSB
of the field. Since there may be some "MSB challenged" engineers in our
midst, we probably ought to specify this.
