CTP Issue 39: Version Number Needed

The issue is that the CTP protocol currently has no version number. If any
incompatible changes in the protocol occur as the protocol develops, it will
be hard for implementations to interoperate and maintain backward
compatibility.

The suggested resolution is to add a version number in the first octet of
all CTP messages and in the CDB, and modify all message format diagrams and
descriptions to reflect this.
