Can I ask you, why would you want to use Outbound in you want forking to occur? Of course, if you don't use Outbound, you may need anohter mechanism to negotiate support of keep-alives: the sip-keep draft provides such mechanism :) But, since I know that you don't think that keep-alive indicators are needed in the first place, I guess there much be another reason why you want to use Outbound.
