We'll also need to deal with scenarios where two calls are joined by a b2bua (e.g., out-of-dialog REFER/Invite with Replaces is terminated by a middlebox). Someone raised this issue over lunch. I think it's not too hard to address unless one or more of the parties don't support Session-ID but others do, and that will be hard.
