I guess IPv4 should have been specified by writing down the header
format, but not the semantics of any of the values, which would not
be "substance".

The substance of "Delivered-To" is private use as a loop detection
sentinel.  The syntax is an implementation detail.  Elevating the
syntax and downplaying the semantics is not progress.
