A revised summary of answers to my questions on snmpEngineBoots are:

   * msgAuthoritativeEngineBoots is 0 in a Request message when the
     authoritative engine id is empty.
        o rfc3414 (USM) Section 3.1 (6) (c)
   * msgAuthoritativeEngineBoots can also be 0 in messages with security
     level set to noAuth/noPriv.
        o The value of this field is ignored for noAuth/noPriv.
        o rfc3414 (USM) Section 3.1.
   * snmpEngineBoots starts from the value 1 when the SNMP agent is up
     and running.
