Thank you for this registration request. Here are a few comments.

You don't need to include your personal mailing address. Also, it's best 
to include a role email address, such as [email protected], in case you 
ever leave the organization.

The Purpose and Syntax sections are pretty much undefined (and the 
Assignment and Security and Privacy sections could be more complete, 
too). Please see more robust examples from other registration requests:
