bug#323: usb hotplug image

After a week of tuning init scripts and tweaking all of the wrong things,
I have a correctly-configured, bootable image that starts up hotplug for
usb and pci.  It could use some testing with usb keyboards and mice before
being integrated into the bbc-contents file (so that it becomes a standard
part of the bbc image). Please ...

	wget http://www.cs.uni.edu/~gray/bbc-hotplug.iso

burn and test.  This image has usbutils integrated, along with the
expanded pciutils (for pci hotswapping) and the hotplug code from
sourceforge.  All of these start up on boot (using rcsysinit).
