As for where it crashes, it is fairly early..but I am not sure exactly
where.  The last few lines on the console are:

RAMDISK:  Compressed image found at block 0
input: AT Translated Set 2 keyboard on isa0060/serio0
VFS: Mounted root (ext2 filesystem).
Freeing unused kernel memory: 184k freed
Warning: unable to open an initial console.
Kernel panic - not synching: Attempted to kill init!
