Apparently 1.9.3 still has read ahead caching disabled, but 1.9.3 did fix
read ahead thrashing.

Now testing code that enables the read ahead cache by initializing the
amount of caching to that determined by CFS and/or PFS.

Done for CFS server.

This was disabled by Zafman or John or someone, but my finding is no
obvious performance hit when enabled - rather an increase in performance.
