The report was not particularly detailed, just a screenshot from an 
iMac, but I suspect this is the same bug going by the symptoms:

Interestingly it is reproducible on 3/3 Xserves here, but with slightly 
different errors from the OFW. I can document the OFW versions if that 
matters.

On one it shows:
