I'm not completely sure what the expected state is, but the files on
mirrors appear to match those in the master copy of the archive, so
this at least doesn't seem to be a mirroring issue.

In that case the correct people to check with would be the ftp-master
team, who manage the archive. You can find methods of contacting them
on https://wiki.debian.org/Teams/FTPMaster ; I've also pointed to this
thread on IRC.
