Well this is back to our leading theory.

Adding
    Port 16 works because it is internally crossed over, due to the
    switch using a new transceiver chip on an old board layout.

I would be nice to understand this problem, but since you have a
work-around (however little sense it makes) we may have to wait for
additional reports to understand the problem.
