Hi,

we can see the following error message in your 'dmesg':

HSF: ERROR: SILICON Laboratories or LUCENT AC97 modem detected!<3>
HSF: ERROR: This driver only supports CONEXANT devices<3>
HSF: ERROR: not a Conexant modem<3>
---

It seems that you have a modem based on a Lucent Technologies or Silicon
Laboratories chipset, and not Conexant.

Linuxant provides drivers for modems based on Conexant HCF and HSF 
chipsets. There is a Linux driver for LT winmodems that might work.
