Another 'device soft reset timed out' error (WN4110B)

I have a box with an Accon WN4110B (Prism54) and i get the infamous
'device soft reset timed out'

This is a limited copy of my dmesg log. I'm using the 1.0.4.3 firmware
version (from prism54.org)

It seems that is something related with the firmware, but it is so
extrange because the card seems standard. Do you know something about
the (rev 01) flag?
