Minor changes to header and long usage info.

Show additional information in the configuration EEPROM:
  - Transceiver type
  - Subsystem IDs and media options for 'B' and later boards.

Allow setting the transceiver type in the configuration EEPROM:
  - changed list of possible media types.
  - changed get_media_index() to differentiate an integer index from a
    text name leading with numbers: "10baseT" is not "10" or 0x10ba.
  - moved the checksum and EEPROM write code to a common section.
  - always use Cyclone checksum location.
