Expand Raid5 now LVM information is partially missing

Hoping someone can help...

I did an mdadm --grow to add another drive to the array. Everything appeared
to be fine during the process no errors. After it was complete (23 hours
later), I rebooted and was unable to find the root volume.

It seems somehow the volumes were converted to lvm. The lvm tools can see
them, just evms can not. In evmsn I can see the md objects, but none the LVM
ones.

I have listed some information regarding the situation below. Advice greatly
appreciated!
