Those personalities worry me! Any reason you are using dm devices
rather than raw disks? I noticed in your first post the device sdj,
which is what evms was bombing on.

Its been a while since a drive dropped from my array but UUU_ tells me
that it is expecting a 4th disk to be in there, but it is not in
degraded mode. Any chance of tearing down the array, zeroing out the
drives, and rebuilding it? I see that its ~800GiB worth of space which
is a pain to backup.

I'm curious to see your partition lists:
