1.) docs: change source from Samba 3.6

This just changes the "Source" from Samba 3.6 to cifs-utils 4.0.

2.) docs: these tools are no longer part of Samba

I'm not sure if this is the best way to deal with the second issue, but
this patch replaces the references to the samba(7) man page with
"cifs-utils" (no man page).
