Did you recently upgrade RPM on your system? I usually get this when the /var/lib/rpm/__db.* files are of a different DB version from the rest of the files in that directory. just removing the /var/lib/rpm/__db.* files (000,001,002,003) and running rpm --rebuilddb will fix it.
