One thing that people are finding with recent versions.  (This won't
solve your first question, but might help with the second.)  Although
cups is a default use variable, judging from some posts on forums (and
my own experience) the following helps.

In /etc/make.conf specify cups in the USE section.  That is, your 
USE= section should contain cups so that it looks like 
USE="cups"

Secondly, cups will install ghostscript as a dependency.  However, after
cups is installed, doing emerge -C ghostscript && emerge ghostscript
seems to be solving various problems.  

Once you get lp0 recognized, hope this helps a bit
