The only thing I can imagine besides a hardware failure is an incorrect /etc/fstab. Did you remember to use the -notail option on the /boot partition, and did you compile ReiserFS support in, not only as a module?

Or does is happen ocasionally, not directly after first reboot? In that case, dunno, good luck.
