I'm pretty sure that's because modules in the kernel that require it 
automatically pulls it in. It's not on by default. A kernel module that 
requires it has to be selected in the kernel for that to be built.

As the binary nvidia drivers are not in the kernel source, it doesn't 
get pulled in via dependency.

I suppose there may be a case for a bugreport to the gentoo-sources 
build - as they could put in an option that pulls in these modules for 
the nvidia driver. However, I don't know if these modules require the 
presence of what gpu driver is selected for their build process (this 
could be a dealbreaker for this option.)

If you use amdgpu, you have to set it as a module. If it's built into 
the kernel, TTM and TTM_HELPER aren't pulled in.

I don't have an AMD GPU in this machine at all, and I remember having to 
set amdgpu to a module to get nvidia's framebuffer to work.
