I guess what comes first would be monitoring of disk-writes/reads.
1.If you happen to have KDE (which I doubt if you have ubuntu) this is
easy:
ksysguard -> File -> New
Now on the left under Sensor Browser click on:
localhost -> Disk Throughput -> "dev-nr. of your HDD"
Now pull any desired sensor to the right
2.gkrellm

can do a similar job
3.sysstat

is a useful command-line tool.
Use something like <iostat -dk 2>
BTW:Did you check MEM-usage (<free>)?
Just a hypothesis:
A proc gone out of control sometimes uses all available
MEM, which *could* lead to continuous swapping, and therefore to
constant disk-usage.
