So that e.g. if you only have vrrp stuff in the config, then the healthcheck thread won't get started (just as when starting it with --vrrp). Or vice versa obviously if you're only using ipvs and not using vrrp. On the other hand I can see that this might be complex to integrate when reloading a config (e.g., when from a config without any ipvs stuff to one with) so maybe it's not worth the effort.
