That's right, use fwmark-ing of outgoing healthcheck packets.

There is a tool allowing to do IPIP encapsulation to different destinations based on fwmark quite easily:
https://github.com/andriyanov/check-tun

It reads keepalived.conf to determine tunnel endpoints and supports config reloading by HUP.
