Your modem being a USB modem, the USB Id's which you quote are useless.

If this modem is recent, currently sold by US Robotics, please try to 
find the box, or copy from the device, absolutely everything which you 
csn see on the device and on the box.


If your modem is one of  models 5635 or 5637, then it definitely is not 
a Winmodem but a modem with a serial controller which does not need any 
driver except the standard Linux serial driver included in each Linux 
distribution by default.

If the model is different, you still can easily try your luck (i.e. not 
a winmodem) by simply applying  the command

sudo  wvdialconf

It your modem indeed contains a serial modem, wvdialconf will find and 
report a modem such as   /dev/ttyS0  or S1 or S2 etc... under the 
condition that you have powered up the modem before looking for it.

You then do not need wvdial to use it.

Use whatever dialler you like , to be configured with the  /dev/ttyS...  
detected by wvdialconf
