Afshin,

scanModem reports that you have an 11c11040 modem.  You may try to
install a driver which may or may not work*  available at

http://linmodems.technion.ac.il/packages/ltmodem/11c11040/agrsm-11c11040_20110811_i386.deb

also download the agrsm-tools package:

http://linmodems.technion.ac.il/packages/ltmodem/11c11040/agrsm-tools_0.0.1_all.deb

You have dkms installed:

Then it will suffice to:
$ sudo dpkg -i agrsm*.deb
to install the modem code and the agrsm-tools packages run
$  sudo agrsm-test
to see if modem is detected.
