[Bug 881] New: Style: separators between parts of a bookset

In the single-file formats, the lack of separator between books in a bookset
looks funny.  Below is a paste from the numbered-text versions showing the
beginning of the Packaging book (you get the same effect in the single-html
version so this is not a text-output effect). This one might not be confusing if
the Appendix weren't empty - should it be?  Or if so, should the section even be
there? Where should Appendices appear in a bookset?
