Document Structure for 3.0

After a fair bit of playing around during the Preview and RC's, I
believe I have now settled on the following overall document structure
for the LSB Core specification. The TOC below is for the generic
document, but is essentially the same for any of the arch specific docs.

NOTE: this is radically different structurally from what went out in
Preview1 ... but despite that it is almost identical technically (i.e.
few normative requirements have changed).

This is a thing of beauty to me ... but beauty is in the eye of the
beholder, so if you think I've made a total train-wreck out of it ...
this is your absolute last chance to say something before the final
document comes out!
