Strange to have a Microsoft type of partition for /boot !? That may be an issue somewhere.

So the command  analyze-my-lvm is failing.

So could you run:
/usr/lib64/mindi/analyze-my-lvm 

It should crash similarly.
Then you can run 
bash -x /usr/lib64/mindi/analyze-my-lvm

and provide us with the results.
