It doesn't sound like a familiar problem.

What brand and model of laptop?  Does the card work ok if you then
eject and reinsert, or is it still misidentified?  Can you be any more
specific about the "sometimes": is it deterministic?  Is it only after
warm boots?  How are you initiating the reboot?  Was the drive mounted
immediately prior to the reboot?
