Linux printing problems

Headed A4 paper is being used to print invoices. Address details are printed in
colour via a colour laser or inkjet printer in a separate run. When trying to
print the invoice details on these pre-printed forms, the address details are
reproduced several times on the same page. This happens on all mono-laser
printers i.e. HP 1100, Kyocera FS1900N and Lexmark, but works OK with a Minolta
colour laser. Can anyone please explain what might be causing this problem.
