There was an O'reilly book on driver development at one time.  It had some
details of PCI drivers development and use of the DMA facilities available.
Some of the PCI interface chip manufacturers such as PLX have reference
designs.  You would then have source for an example driver.  The ones from
PLX are very good.  Here are a few links.
