a solution that works is:
Mailscanner + F-Prot, or Mailscanner + Sophos

it's a cake to install and runs without problems!!!
(one server has and uptime of 14 months and manage about 2000 emails per 
day, some with big attachments).

f-prot is more expensive, 300 $ per year, but doesn't restrict the use 
for scanning email.
sophos works good, but f-prot is EASIER to keep updated.
