Hmm did you fill out all the questions? It could be the radio button 
ones really want you to fill in something. Could you fill out 'don't 
know' for each if you want to skip it?

Or, if you still have it open (I realize probably not likely at this 
point), if you'd be willing to copy & paste your answers and email to me 
that would be great.
