FWIW, I've seen this happen when upgrading a system en masse (things like
rpm -Fvh *rpm 
in a directory containing ALL of the rawhide RPMs); rpm sometimes hangs in
infinite select() loops, and it seems to upgrade just fine, except the
record of the old package is retained.  It's a PITA to deal with later.
