Maybe you will be interested by the quick set of macros I wrote for typesetting right-to-left with Groff and the -mm macros: http://baruchel.free.fr/~thomas/code/rtlgroff.pdf The greatest things: right-to-left running over page breaks without disturbing the header, and even... running over page breaks in two column mode!
