You are not missing anything, it's a bug (or at least a bad feature).

Once you switch to KDE 3.2, that won't be a problem. But, as far as I can 
remember, you need to run kaffeine with --nofork option, or it was something 
like that. Check "kaffeine --help-kde" and "kaffeine --help" for more info.

In Konqueror, go to Settings / Configure Konqueror / File Associations, and 
than change everything you use kaffeine for to start as "kaffeine 
--nofork" (I think that was the option, but I can't test it with KDE 3.2, 
check with help).
