getting ftape 4.05 to work

I'm trying to get ftape (version 4.05) to work, but I/O errors keep me from a 
resulting working 'something'

It seems that the rewind command won't work for some or another reason.

I've got a Ditto max prof with Sony ditto 2GB cartridge ( preformatted 
replacements), with accelerator set at base address 0x200 (no other place is 
free), interupt 6 (5 won't work for some reason) and DMA channel 0.

It will return it's status (including the message that it has loaded the 
tape). It will write a volume table, but won't rewind. 
Writting to the tape will start but stop with the message "permission denied"

Any suggestion what might be wrong? The accelorator perhaps?
