Random keyboard failures

In our computer lab with LTSP fat clients, we experience a growing 
number of keyboard failures during runtime. (The teacher says it has 
become more frequent.)

All clients boot flawlessly, but "once in a while" keyboard failes on 
one of them, then maybe on another one. These failures are not limited 
to certain clients, they can occur everywhere, but I have only had 
reports of this one lab yet.

The possible range of reasons is rather big. We use PS2-to-USB adapters 
which are usually very reliable. But adapters increase the risk of 
contact failures, so there might be one reason. Or the adapters fail 
randomly.

Whenever this occurs, it is enough to plug off and on the adapter, 
either on USB or PS2. The keyboard reboots, and everything runs well 
again. The point is, when this happens during a test situation, it's 
very annoying for the student.

Maybe there are more people here who have similar experience? Where 
would you start to find out the reason?
