LTSP NFS might be what you need for your use case : installing packages on LTSP and making them instantaneously available on running clients. However I don't know much about it.

Exact, then your users will get the new LTSP image (with skype) on reboot. Running 'ltsp-update-image' while there are connected users works fine, although it uses a lot of the server's CPU. 

If you want to install packages client side while the users use it, I think you can also connect to the clients using ssh (see parallel-ssh or ansible for batch mode) and get elevated priviledges (sudo). However, as you noticed, these changes will go away on shutdown.
