I put a kernel log of another disconnect to

  http://www.hut.fi/~apm/kern.log

with ehci-hcd compiled with DEBUG and some dump_stack() calls added.
So the token field indicates transaction error in qtd_copy_status(),
right? How could I diagnose further?
