The Windows log looks normal.  By the way, you didn't have to send the 
entire log file!  The first 10 URBs would have been enough.

I don't see any reason why the Linux driver shouldn't have worked.  It 
sent exactly the same packet contents as Windows did (except for the 
Tag value, which doesn't matter).  It makes me suspect something else 
is going wrong.

Can you get a usbmon log under Linux?  Instructions are in the kernel 
source file Documentation/usb/usbmon.txt.
