NETSCAPE3.0:IMAGE emits the HTML rather than returning it (it happens to
return #\>).  This probably does what you want:

(ns2.0:with-anchor-noted (:reference "a url" :stream stream)
  (netscape3.0:image "/cl-http/icons/blueball.gif" "alt" :stream stream))
